Medical Records
Your medical record contains information about your health history and the care and treatment you received during your stay at White Mountain Regional Medical Center (WMRMC). The medical record must be completed by all physicians involved in the patient's care. Patient related information may be released to a physician or medical facility for follow-up and continued care of the patient when needed.
Your medical record is a confidential document, so your written authorization is required before a copy can be released. An authorization form may be obtained from the Medical Records office. A copy fee may be required. For more information contact the Medical Records office at (928) 333-7141.
