What is a Swing Bed?
Swing Bed is 24 hour skilled nursing care plus the benefit of “rehabilitation”
therapies to help patients transition to home or a long term care facility.
Swing bed can also be utilized to help rehabilitate from surgery, illness or accident.
Swing Bed is like a bridge from the hospital before going home. It gives the
patient time to heal and adjust before returning to everyday life. Our
main purpose will be to provide you the necessary care through a collaborative
team approach, facilitating therapists and skilled nursing to obtain the optimal
outcome possible.
Who needs Swing Bed care?
Patients who need rehabilitation after orthopedic surgery or fractures
Stroke Patients
Patients receiving IV antibiotic therapy
Patients needing continued wound care
Patients needing to regain strength and mobility
To qualify for Swing Bed care, a Medicare recipient must have completed a three
day acute care stay, either at WMRMC or another acute care facility.
Who Pays for Swing Bed?
The Swing Bed program is a hospital based program and Medicare
Part A is the primary payer of Swing Bed services. Medicare can cover Swing Bed
for up to 100 days with various co pay charges. Co pays can be offset by
secondary insurance since health policies differ our admission coordinator will
verify your coverage, so as long as a patient meets Medicare criteria, has days left in the benefit period, and the doctor approves the stay, a patient may receive Medicare Swing Bed services.

Physical, Occupational and Speech Therapy are offered for patients that have had
joint replacement, major orthopedic surgery, or stroke, thereby improving their level of capability, independence and function.

The patient’s personal physician, or a hospital based physician, called a
hospitalist, who keeps the patient’s doctor informed
A case manager, who facilitates patients stay, coordinates the patient’s care
and discharge

Case Management
As your case manager, I am here to advocate for, you, the patient. I make referrals and communicate with your insurance
company, keep you aware of your clinical goals and plan for your discharge.
Additionally, I coordinate and lead your weekly interdisciplinary team meeting
with your healthcare team to develop, reassess and update your plan of care. I
understand how important your family is to your recovery and encourage families to participate in these
meetings. I am here to answer any questions before, during, and after your stay.
